Please take a few minutes to read it carefully.Our office is open from 9 a.m. to 4 p.m., Monday through Friday. Patient appointments are usually scheduled on Tuesday and Thursday, and Monday, Wednesday and Friday are generally reserved for surgery. You can

(203) 384-4500 during regular office hours. When your condition requires urgent attention, we will make every effort to meet your needs. If you need to cancel an appointment, please let us know as soon as possible to allow other patients the opportunity of all available appointment times. We offer FREE PARKING for all of our patients.
The following forms are provided for convenience. These can optionally be filled out prior to visiting the office.
For your own benefit, please make non-emergency calls to the doctor during regular office hours so that your medical records will be available. Our receptionists will take preliminary information and expedite a return call from the doctor or a member of our staff. Refills for medication will only be considered during office hours and may require up to 24 hours to be approved. Medications will not be refilled on the weekends so please plan accordingly.
